Set up Automatic Donations with Printable ACH Form:Use our Online ACH Form or download the PDF file to set up automatic donations through Automatic Bank Withdrawals. Click "Download File" below to download our ACH Authorization form. Fill it out online, print and sign it, then mail it to the address below or give it to one of our board members.
Avoid Transaction Fees: Mail a Check:Most online services, such as PayPal, charge a 3-5% transaction fee. Donations by check are one way to make sure all of your donation goes directly to House of Mary. (see mailing address below, or you can always give your donation to one of our board members).
